Sports Physicals for the New School Year Start April 1
Sports physicals for the new school year may be performed on or after APRIL 1. You can schedule an appointment with your family physician or at the School-Based Health Center (651-2100). If your child is not registered at the School-Based Health Center, please complete the Consent and Registration forms.
The deadlines for sports physicals are August 1st for Fall Sports, November 1st for Winter Sports, and February 15th for Spring Sports. The athlete must be cleared to participate on or after April 1, 2024, based on a physical examination conducted within 12 months of the signature of the provider. The clearance is valid through the first day of Fall practice of the following year unless a re-examination is required. Here is the 2024-25 DIAA Athletic Physical and Consent Form.

CSW has contracted with ArbiterSports to manage our over 800 DIAA physicals on a customized digital platform. Each CSW family will create a family portal to register for sports and upload their sports physicals. The platform is FERPA compliant. We are excited because our coaches and athletic trainers will have access to a mobile app that holds emergency information for their team rosters at their fingertips.

You will need 15 minutes to create an account, complete the form, and upload your physical paperwork.
All athletes are required to provide a 2024-25 DIAA physical.
Make sure you have your child's doctor's phone number, insurance information, and any medications your child needs to get started.
CSW has adopted procedures to care for your child when they become sick or injured at school. The Consent to Treat form will also be completed at this time. Your student will have to sign this form, too.

Look for an email from the school nurse with instructions on how to complete CSW's Emergency Treatment Form online.
All incoming freshmen will receive an email from the School Nurse with physical information and other required medical forms.
If you still need a physical, The School-Based Health Center will reopen the week of August 12th.
